Health is the three nyépa each at their proper level. Push one humor toward excess below and watch how it acts on the other two — not evenly, and not always in the same direction. Leave it unaddressed and the tradition's own severity ladder applies: a single humor disturbed, then two combined, then — rarest and most serious — all three at once.

Wind and Bile now sustain each other — Wind's erratic drive keeps agitating Bile's heat, and that heat and dryness keep feeding Wind. Harder to settle than either alone.
A cold, heavy, obstructive pairing — Wind's weakened digestion feeds Phlegm, and Phlegm's blockage of the channels keeps deranging Wind's movement. A classically common, stubborn combination.
Heat and cold locked in direct opposition — each humor is actively depleting the other rather than merely coexisting, which tends to show as an unstable mix of hot and cold signs together.
The rarest and most serious classical category — all three humors disturbed at once. Considered the hardest presentation to treat, since correcting any one humor risks aggravating another.
